If the applicant is a Parent, Guardian, or Custodian:</p> <ul> <li>Presentation of an ID card or other suitable identification document and proof of the relationship;</li> <li>Presentation of the ID card or other suitable identification document of the certificate holder.</li> </ul> <p>3. If the applicant is a Proxy/Representative:</p> <ul> <li>Presentation of an ID card or other suitable identification document;</li> <li>Written authorization from the certificate holder, identifying the proxy/representative and specifying the purpose of the certificate. This declaration has no expiration date and remains valid for the specific act;</li> <li>Presentation of the certificate holder's identification document.</li> </ul>
