• Accompanied by two authorized adult witnesses.

For citizens under 12 years:

• Presence of the minor with their Citizen Card;
• Presence of the person exercising parental authority, guardianship, or curatorship with their Identification Document;
• Document proving the exercise of parental responsibilities or guardianship, issued within the last six months (if applicable);

If the child is of parents who are married or in a de facto union:

Presence of the minor with their Citizen Card, accompanied by one parent with a valid identification document.

To prove a de facto union, a joint declaration under oath by both members of the union, stating that they have lived in conditions analogous to marriage for more than two years, must be presented.

If the child is of parents who are divorced, legally separated, separated in fact, or have never lived in a marriage-like relationship:

Presence of the minor with their Citizen Card, accompanied by the parent exercising parental responsibilities, with a valid identification document and a document proving the exercise of parental responsibilities (birth certificate or updated sentence or agreement certification specifying these responsibilities).

For interdicted or incapacitated citizens:

• Presence of the interdicted/incapacitated individual with their Identity Card/Citizen Card;
• Presence of the person exercising guardianship or curatorship, with their Identification Document;
• Document proving the exercise of guardianship/curatorship, issued within the last six months.

Urgent Request:

• 3 days for Mainland Portugal;
• 4 days for Madeira and the islands of São Miguel, Faial, Pico, Santa Maria, and Terceira in the Azores;
• 5 days for the islands of Graciosa, São Jorge, Flores, and Corvo in the Azores;
• 5 days for Europe;
• 7 days for the rest of the world.

Extremely Urgent Request:

• Delivery on the same day (for requests made by 11 a.m.);
• Delivery the next day (for requests made after 11 a.m.).

Validity

• Until the age of 25: 5 years;
• After the age of 25: 10 years;
• Brazilian citizens benefiting from the Equality of Rights and Duties Statute under the Porto Seguro Treaty: Validity corresponds to the Residence Card's expiration date, not exceeding the period set by the citizen's age.

Assignment of a New Civil Identification Number

It may be requested in the following cases:

• Identity Theft;
• Forgery;
• Use of Another Person's Document.

And provided that:

• Documentary evidence is provided, i.e., through an Order from the President of the Board of the Public Registry and Notary Institute.
• The respective identification document is valid.
